for Health Data Science

Students and researchers in the health sciences are faced with greater opportunity and challenge than ever before. The opportunity stems from the explosion in publicly available data that simultaneously informs and inspires new avenues of investigation. The challenge is that the analytic tools required go far beyond the standard methods and models of basic statistics. This textbook aims to equip health care researchers with the most important elements of a modern health analytics toolkit, drawing from the fields of statistics, health econometrics, and data science. This textbook is designed to overcome students’ anxiety about data and statistics and to help them to become confident users of appropriate analytic methods for health care research studies. Methods are presented organically, with new material building naturally on what has come before. Each technique is motivated by a topical research question, explained in non-technical terms, and accompanied by engagingexplanations and examples. In this way, the authors cultivate a deep (“organic”) understanding of a range of analytic techniques, their assumptions and data requirements, and their advantages and limitations. They illustrate all lessons via analyses of real data from a variety of publicly available databases, addressing relevant research questions and comparing findings to those of published studies. Ultimately, this textbook is designed to cultivate health services researchers that are thoughtful and well informed about health data science, rather than data analysts. This textbook differs from the competition in its unique blend of methods and its determination to ensure that readers gain an understanding of how, when, and why to apply them. It provides the public health researcher with a way to think analytically about scientific questions, and it offers well-founded guidance for pairing data with methods for valid analysis. Readers should feel emboldened to tackle analysis of real public datasets using traditional statistical models, health econometrics methods, and even predictive algorithms. Prior to the respective care practices, the authors describe the core of personalized care and illustrate its connections to value-driven care and evidence-based medicine. The last part of the book addresses organizational and other conditions under which personalized care can thrive. In the Netherlands, substantial work is being done on value-driven care and more is currently being discussed. This book focuses on the benefits of value-driven care for patients and healthcare providers alike. To do so, it explores a range of representative cases and scenarios, such as: Scenario 1: You are able to surgically remove the most difficult tumors and find out one day that you don’t need to do so in all indicated patients. Later you can find out how to recognize when surgery isn’t the best treatment for certain patients. And now your team hasa fixed routine to find the optimal choice together with the patient. Scenario 2: You consider yourself to be a patient-centered doctor who is really in touch with their patients – until you ask them to report for themselves on how they are doing. In the "patient-reported outcomes" you see that you have completely missed an important aspect. Later, you can structurally integrate this into your outpatient clinic. Scenario 3: In line with the planning principles of your hospital, you see chronic patients once a year – and of course also ''as needed'' – until your data shows you that you see patients too early or too late, but rarely when it would be most helpful. Later you can discover how to offer your consultation exactly on time – based on early signals and often even before the patient calls the clinic. This is what value-driven care is all about: care is of more significance to the patient when it is tailored to his or her unique situation. Recognizingthe patient’s needs, preferences and values, and structurally adapting care accordingly – that is the noble profession of value-driven care, which takes us an important step closer to the ideal of personalized care.

Care for older adults in Emergency Departments (EDs) has historically been focused on acute medical conditions with little emphasis on geriatric-specific issues. In 2010, emergency departments (EDs) throughout the nation saw almost 130 million patients, 15% of whom were 65 or older. The number of older adults who visit an ED has doubled in the last decade and continues to grow rapidly. Older adults receiving care in an ED are highly likely to be admitted to the hospital, much more so than their younger counterparts. Preventing a hospital admission saves older adults from frequently encountered adverse events, including hospital-acquired delirium, functional status impairment, cognitive loss, and nursing home admission. It is unknown how many older adults are hospitalized for reasons other than acute medical illness, such as functional decline, polypharmacy, progressive dementia, caregiver stress, or unstable living situation. These non-emergent conditions are rarely addressed during a typical ED visit due to lack of resources, significant patient volumes, and the need for rapid turnover of care spaces. The predominant management strategy of emergency physicians to handle these important but not imminently life-threatening geriatric issues is to recommend hospital admission. Northwestern has pioneered the Geriatric Emergency Department through the creation of the Geriatric Emergency Department Innovation model (GEDI), with goals to prevent admissions for older adults by assessing and meeting their geriatric-specific, non-acute care needs in the ED. The GEDI model at Northwestern centers on a multi-disciplinary curriculum composed of clinical, didactic, and practical arms developed by emergency medicine and geriatrics educational experts. In this title, we will present case studies of older adults seen in the Emergency Department through the GEDI model and discuss means of identifying/screening for, diagnosing, and treating geriatric syndromes seen in the emergent care of the older adult patient. In addition, there will be a set of concise “take-home points” for each case study that will be easy to commit to memory and implement in clinical care of older adults. As the number of seniors seeking emergent care will continue to increase, the ED setting must become responsive to geriatric-specific needs. This book will provide a variety of models detailing how to offer comprehensive, state-of-the-art, optimal care for managing the full range of geriatric syndromes that regularly present in the emergent care setting.

This title is an easy-to-read guide outlining specific differences in communication, clinical therapies, medications, protocols, and other critical approaches to the care of African Americans. The book discusses a wide range of disorders impacting African Americans and takes a comprehensive and evidence-based approach to the clinical support of providers that see African American patients. Recording the worst medical outcomes of any racial/ethnic group in America, African Americans have the highest mortality, longest hospital length of stay, worst compliance with medications and referrals, and the lowest trust of the healthcare system. The increased life expectancy of the population since the early 1900s had been built on the improvement of living conditions, diet, public health and advancement in medical care. With this we have seen a steady decline in the age-specific prevalence of vascular and heart diseases, stroke and even dementia. In addition, societies worldwide struggle to develop a large enough workforce to treat aging patients, which forces geriatric patients to rely on physicians in a wide array of specialties that are often not trained for their demographic. These trends have created a tremendous need for trustworthy resources, yet with regard to hypotensive syndromes, nothing of this nature exists. Hypotensive syndromes represent a heterogenous group of disease states. Hypotensive syndromes are characterized by low blood pressure following postural changes, meals and neck turning. These are common conditions seen in the elderly and could be due to blood pressure dysregulation. These syndromes frequently cause dizziness, syncope and falls in the elderly as well as a resultant decrease in function, and they are frequently mistaken for other conditions. This is especially true among physicians who are not trained to consider the unique needs of an aging patient. The proposed book is designed to present a comprehensive approach to the management of hypotensive syndromes in the elderly. Currently there are no guidelines or good resource to guide about these conditions. This book will also discuss the challenges of diagnosis and management of these conditions. The text introduces the concepts to set a clear foundation before covering the syndromes as they present in other comorbidities, including diabetes, heart failure, and a wide array of serious conditions that are common in older patients. Subsequently, self-management was applied as formalized programs for a variety of populations and health issues. In reflecting on self-management, it is important to note that it would be difficult for individuals not to be aware of their specific health behaviors, which could include unhealthy behaviors. As self-management has evolved, essential skills identified include behavioral modeling, decision making, planning, social persuasion, locating, accessing and utilizing resources, assisting individuals to form partnerships with their health care providers and taking action. These are key skills that would benefit health professional educators, clinicians and patients. This book, consisting of three parts, provides insights into the aspects of self-management as it relates to its definition and application. It highlights how self-management can be applied to various long-term health conditions, for different populations or target groups and in different contexts. The text provides an overview of self-management and the rationale for its applications by illustrating its use in specific clinical conditions and in different sub-populations and target groups. Academics can use the book as a textbook when teaching postgraduate and undergraduate students about self-management as a technique to facilitate community reintegration for individuals living with long-term conditions. With these large accumulations of developments in the field of nutrition, the need for a new edition of this book is obvious. Our vastly improved nutrition knowledge gives us the capability of preventing a sizable fraction of the chronic diseases that afflict the people of our world, but only if these discoveries can be translated into effective action at the population level. Nutritional Health endeavours to address the needs of those who would most benefit from up-to-date information on key areas in the field of nutrition. The book starts with a discussion of the nature of nutritional research then moves into an overview of the most important aspects of the complex interactions between diet, its nutrient components, and their impacts on disease states, and on those health conditions that increase the risk of chronic diseases. Parts three and four discuss diet and include new chapters on the Mediterranean diet, the DASH diet, the flexitarian diet, and the low-carbohydrate diet. The final two parts of the book discuss emerging trends in nutrition science, such as gut microbiome and sustainable diet, and areas of controversy in nutrition, such as the influence of the food industry and dietary supplements. Though there is a broad consensus within the nursing profession on the importance of health promotion and the promotion of self-management, nursing professionals often struggle with the underlying theoretical and practical aspects involved, as well as the right type of intervention to use and how to evaluate the results. The book departs from concepts of health. In the first chapters, it examines health at the micro-, meso- and macro-level, combined with epidemiological health indicators. The following chapters focus on prevention, health promotion and self-management, while also broadly discussing prevention. The book then turns to the development and purpose of, as well as plans for, health promotion and health education. The Intervention Mapping protocol is the starting point, addressing questions such as how to motivate a patient to other, healthier behavior. Subsequently, the field of health promotion is extended to disease prevention, patient education and self-management. On the basis of patients’ needs, the book describes methods and interventions to promote self-management in detail. Self-management and chronic health problems are also highlighted, along with the patient’s social network in connection with self-management and eHealth. Lastly, the book explores the relationship between nursing and health promotion, as well as disease prevention, diagnoses, interventions and care results.Additionally, this edition includes two trainings on ''Promoting the self-management of the patient '' and on ''What is the role of the nursing professional in promoting self-management of the patient?''. RTD is an advanced oncology procedure that involves the procurement, for research purposes, of “fresh” tissues within two to six hours of a cancer patient’s death. Since RTD involves the retrieval of tissues after death, and since the collected tissues are of great importance for medical research, the need for any form of informed consent to regulate this procedure has been questioned. This book argues for the necessity of informed consent to govern RTD, and it provides the reader with a bespoke informed consent process applicable to cancer patients. The analysis unfolds at the intersection between applied ethics, public health ethics, and clinical ethics, and it is informed by philosophical theories of informed consent and by the social implications of individual choices. By viewing medical issues relating to informed consent in oncology from an ethical perspective, the book combines philosophical analysis with discussion of concrete cancer-related issues. As a result, the book is suitable for readers interested in ethical reasoning as well as for those with a medical background. Many of us have elderly relatives who died from a drug-resistant infection, and some of us have suffered from a resistant urinary infection that likely came from intestinal bacteria following antibiotic consumption. Antimicrobial Resistance in the 21st Century provides a broad introduction to the subject in which the situation with problematic pathogens is detailed, the biology of resistance is described, and gaining approval for new antibiotics is discussed. Some topics are immediately practical, such as watching for resistant pathogen sub-populations in cultures taken from patients; other topics point to future research efforts that may lead to new antimicrobials and ways to stimulate the action of existing ones. Chapter two addresses the role of carbohydrates in inexpensive and tasty foods, while chapter three outlines how vegetable fats, notably palm oil, are used in selected industrial foods and explores their relation to hyper-palatability and other health issues. Lastly, the fourth chapter discusses the increasing demand for high-protein foods and the concomitant availability of vegan products. This brief is of interest to researchers in food production, food hygiene and public health, but is also relevant for certification bodies and specialists in the food industry.

This volume provides readers with a systematic assessment of current literature on the link between nutrition and immunity. It challenges the myths, beliefs, dogma, and customs of suicide from the earliest theories. It offers fresh insights into dark spaces. World-wide, suicide deaths are three times greater than homicides, and are increasing. Current approaches to stem this ‘epidemic’ are not working, or have very limited success. Mental health interventions, theories about a suicide or a depression gene, and the ever-increasing dispensing of antidepressants have not lessened the stark statistics. The authors attempt to understand the soul of the suicide — addressing the social, economic, political, historical, geographic, and cultural contexts in which suicide occurs. The social order is indelibly connected to settings, places, circumstances, relationships, occupations, climate, and milieus. Most of the 36 diverse categories of self-motivated deaths defy a ‘one-size-fits-all’ approach. Recognising contexts and looking outside the confines that have imprisoned thinking about suicide, could well be more effective in alleviating or mitigating suicide than years searching for a possible vaccination against such death. The book is an appeal to move beyond the medical model of suicide. Written in a very accessible style, it is of interest to social scientists, philosophers, professionals and researchers in public health, medical and behavioural sciences, and lay persons alike.
